This class/classes are in the same payment/enrollment period of the course I am dropping. I am NOT withdrawing from Bethel University. ______________________________________________________________________________ _________________________ Student Signature Date ******Notice from the Financial Aid Office****** The Financial Aid office is required to obtain a letter of intent from the student when they drop a course to confirm that they are planning to continue attendance. If a notice of intent is not received within 7-days Financial Aid funding amounts could be affected by the drop or non-attendance. The date of continued enrollment must also start within 45 calendar days after the end of the module the student ceased attending or the student will be considered withdrawn for that payment period.

Did the student complete a Notice of Intent to begin a module in the same payment period within 45 days of the last module completed ? EXAMPLE A student enrolls for one three-credit course in each of the three modules. Student ceases attending module 1 after completing 10 days. The student does not confirm attendance in a later module. The student does not attend module 2, but returns for the one 3-credit course in module 3. The student withdraws from module 3 after completing 15 days. Our issue on this was how to correctly count the days the student attended and use the R2T4 web calculation sheet. Could not use the web sheet because it was based on calendar days. I asked on Finaid-List if anyone had a R2T4 calculation sheet in Excel and I received several responses. I found one that worked well for us that I could put in the number of days scheduled to attend and the number attended to obtain a calculation rather than performing a hand calculation. The correct calculation on this scenario can be found in the 2012-13 FSA Handbook, Volume 5, Case example Six and Seven.
