  2. What is End of Life Care? • Anne Porter – cardiac network • Wendy O’Connor – cardiac network • Early 2011 • Advanced Care Planning (ACP)? • Preferred Priorities of Care (PPC)? • Advanced Communication Skills Training (ACST)? • Gold Standards Framework?

Progress • Initial meeting with Elaine Owen May 2011to discuss “End of Life Care” for patients who have Heart Failure: • Devised a questionnaire, addressing Skills, Knowledge and Attitudes of Staff • Rolled it out to Heart Failure Specialist Nurses (HFSN’s), Cardiologists and GP’s • Presented the findings and recommendations to the Cardiac Board

Recommendations • Provide Advanced Communication Skills Training (ACST) to the MDT, particularly the Cardiologists • Devise an e learning programme for the MDT • Scoping exercise - how many patients have been given an opportunity to discuss plans for end of life care, as a baseline? • Are preferred priorities for care documented? • Are they shared with the MDT? • Review baseline evidenceafter 6 months • How important are prognostic indicators?

Where are we now?/ Achievements • Event for the MDT in March 2012 • Date set for ACST in October 2012 • E o L Care Training Menu almost complete • Questionnaire drawn up to establish a baseline of current practice in relation to ACP’s and PPC and distributed to HFSN’s • Meeting arranged to discuss Prognostic Indicators with Heart Failure Nurse Specialists in July • E o L is now part of the Cardiac Work Programme

  8. Collaborative work • Internal Cardiac Defibrillators (ICD) • Protocols and pathways. • Responsibilities. • Service of care available. • Timely communication.
