  2. Where are we now? • Last year, BOS: “come back with 3 options” next Spring • Since then: Dr. Mark Meiners assists w/strategies development: #1 Network of Care #2 Physician Strategy #3 HSD Health Plan/Pilot Projects

FY 2003-04 Consultant Team • Program Development consultant: Dr. Mark Meiners, UMD, MMIP • Administrative Action Plan (AAP): Mercer Human Service Government Consulting • Deliverable: AAP that identifies and describes the necessary program design elements to guide in preparing for and implementing LTCIP (due June 30, 2004)

  5. Administrative Action Plan • Detailed description of “vision” • How to get there from here • Phase-in timelines • Resources needed • Responsibilities (I.e., Feds, State, CO.) • Draft to stakeholders by may 2004 for comment

Network of Care • Beta testing with • consumers and caregivers • community based organizations • other providers, Call Center staff • To develop “continuous quality improvement” program/ Community Education Workgroup • Enhance use of NOC as communication tool • Measure behavior changes of providers and consumers • Update: Funding Letter of Intent in to RWJF (1-04)

Physician Strategy • Awarded planning grant from California Endowment: $142,000 • Partner w/physicians vested in chronic care • Develop interest/incentive for support of “after office” services (HCBC) • Identify care management/MD incentive resources • Train on healthy aging, geriatric/chronic disease protocol, pharmacy, HCBC supports • County Medical Society as partner • HeartParnters opportunity

  8. #3. Health Plan Pilots • Pilots to do small, voluntary models of care integrated across the health, social, and supportive services continuum: • Examining MassSCO-type model for Healthy San Diego Health Plans+ to develop pilot (consultant support) • Evercare demo for “duals”

Next Steps • Obtain NOC strategy funding • Initiate Physician Strategy (w/Pacificare?) • Explore MassSCO model in San Diego env. • Develop Administrative Action Plan to continue moving forward on 3 options (3-04) • Stakeholder consensus on AAP (4-04) • Board of Supervisors approval for AAP (and state funding application?) (5-04) • Continue working w/OLTC for funding, waivers, support w/other state depts.
