General Information • Use the accrual method of accounting • Round all financial data to whole numbers • Submit separate data for each county • Include only information from the CTC, Operator, Coordination Contractor, or a School Bus agreement • Fixed Route only if purchased through CTC, but do not include trips delivering meals
How to Use The Forms • Click “Save Form” before leaving each section • Clicking “Save Form” will check for errors, calculate totals, and save changes • Greater than a 10% change requires justification
Section I: Face Sheet Instructions • Period Cover – July 1 to June 30 • Network Type: • Complete Brokerage – provides no transportation • Partial Brokerage – provides some transportation • Sole Source –provides all transportation
Section II: Operating Data Instructions • Fixed Route (Bus Passes) • One Way Trips (Tickets): one (1) trip per pass • One Way Trips (Weekly): ten (10) trips per pass • One Way Trips (Monthly): forty (40) trips per pass • One-Way Trips – by Funding Source • Agency for Persons with Disabilities (new)
Section II: Operating Data Instructions • Unmet Trip Request – may be used to justify future need and additional funding • Complaints – include complaints referred by the CTD • Commendations – do not overlook
Section III: Financial Data • Agency for People with Disabilities (New) • Agency for Workforce Innovation (New)
Closing Thoughts • Communicate with Contractors • Try to report the information as early as possible • Be prepared to explain increases or decreases greater than 10%
