Queensland Diabetes Clinic (QDC) • Operates inside the Mater Health facility providing services for Endocrine and Diabetes. • The service covers Public and Private sectors from Paediatrics through to older persons inclusive of Mothers and Indigenous Outreach.
QDC Statistics • March 07 Monthly Clinics • 57 clinics • Covering 200 hrs • Scheduling > 550 • DNA 80+ (15%) • Historically DNA’s spike up to 50% for some clinics
SMS Technology • Use of SMS to communicate has become a reality due to the Australian public embracing phone technology over the past 10 years. • Generation X and Y have widely embraced communication technology making them a perfect candidate for SMS health communication.
A Previous Successful Improvement Strategy Aim of Project: To determine if use of SMS reminders decreases DNA rates within an Outpatient Clinic Improvement Sought: Reduction in DNA rates for trial clinics. What Areas: Young Adults and Adolescent Diabetes Clinic, Queensland Diabetes Centre. By When: May 2007. With What resources: Clinic Manager processed SMS once a week taking less than 10 minutes. Some custom report generation was required.
Key Changes Implemented • Use of SMS software to create custom message template and send SMS messages. • Reply SMS messages to clinic via email • Email client based SMS back to patient for ongoing communication post initial confirmation
Outcomes so Far • There has been a decrease in DNA’s, from 28% to 4% over 3 months for Young Adults Diabetes clinic related to confirmations via SMS. Period of SMS Trial
Outcomes so Far • Early evaluation show 5%-8% improvement for DNA’s for Adolescent Diabetes clinic related to SMS as procedure (Phone calls) for confirmations was already in place. Period of SMS Trial
Lessons Learnt • SMS confirmations can be conducted in a timely and efficient manner • Over 50% of patients contacted via SMS responded with SMS • A decrease in DNA’s was noted for the trial period in the Young Adult Clinic • SMS prompted Patient to reschedule and/or update Demographics i.e. Address
