He felt pain as he twisted his left knee but continued to work. The next day the pain was worse so he went to hospital. The assistant driller was using a hoist to lift a Kelly hose but the hose snagged on a bolt securing the hand tools board. He reversed the hoist at the same time the roustabout tried to release the hose manually and his middle finger trapped against the hand tools board . Can the task be done using ‘hands off’ principle? 6. PDO Incident First Alert – OFSAT What happened A 27 year old yard crewman was standing on top of casings on a 40 foot trailer whilst he rammed in chocks to secure the dunnage. He felt dizzy, lost his balance and fell 1.8m to the ground. He painfully fractured his right wrist and grazed his face and leg. Mr. Musleh asks the questions of can it happen to you? PDO Incident First Alert – Al Hajiry What happened A 44 year old mason was unloading glass reinforced concrete panels from a trailer and onto a trestle when one of the panels inadvertently fell towards him impacting on his left leg and shoulder resulting in a fracture to his left leg. PDO Incident First Alert – BaOmar What happened A 40 year old Well Head Technician (WHT) was manually aligning the corrosive insertion ring while instructing the Assistant Driller to lower the tubing hanger. As the hanger was lowered the WHT right index finger became trapped , he reacted by pulling the finger out causing the amputation of the finger tip exposing the bone. PDO Incident First Alert – Shi-Vani Oil & Gas Co What happened A 32 year old derrick man had just fixed a small leak in a connection between the shale shaker pipe and the flow line (which was also connected to the BOP).  He checked the size of the gap between the two pipes by sticking in his finger.  Vibrations from the BOP caused a sudden movement in the pipes, the gap closed and his index finger was crushed. 10. PDO Incident First Alert – WGCCC What happened A 53 year old tipper driver driving an articulated tipper truck laden with sand took a short cut up a steep ramp. He was forced to brake when he missed his gear and the trailer rolled back down the ramp and overturned causing the cab to lift off the ground. The cab dropped back down to earth when the pin connecting the tipper to the cab snapped. The impact fractured the drivers pelvis. 11. PDO Incident First Alert – Al Turki What happened A 31 year old operator was laying out 6” flat rubber hoses which were coiled on a large horizontal reel on a hose deployment unit. The reel jammed as the last part of first hose was nearly uncoiled. The operator managed to free the reel but in doing so the motorised reel began rotating again uncoiling the last loop of the hose. This caused the metal coupling on the end of the hose to fly off the reel at speed and it hit the operators head, killing him. PDO Incident First Alert – KCA Deutag What happened A trainee assistant driller needed to move a hose to the inspection area and so he hooked it to the forklift using two chains connecting the boom to two end of the hose. The forklift drove to the inspection area and as it lowered the hose one of the chain hooks slipped causing the hose to fall and swing striking him on the foot causing a fracture. PDO Incident First Alert – NDSC Hand position before loss of grip What happened During a routine inspection of a back up engine, the Chief Mechanic put his right foot on the skid 40 cm above ground and was holding the cross bar with his right hand. His right hand slipped from the cross bar and he lost his balance falling to the ground landing on his left hand fracturing his left wrist. As he pulled one of the barriers his grip slipped and the barrier struck him on left thumb causing a fracture and a deep cut. Mr. Musleh asks the questions of can it happen to you? 18. PDO Incident First Alert – Ardiseis What happened A truck driver was using a chain binder to secure a load on his trailer. He pushed the lever down to lock it in place using a cheater bar but the chain binder had not fully locked and as he removed the cheater bar the lever under high tension swung up violently and hit him in the face causing severe injury to his right eye. PDO Incident First Alert – Powertech What happened A contractor was de-coupling a flexible hose in preparation for a rig move when the residual pressure in the hose caused it to whip up and hit him in the face causing a fracture to his jaw. A roustabout went to move the tongs so they would not hit the tubing elevator but in doing so his fingers became trapped between the elevator and tongs fracturing his finger. 21. PDO Incident First Alert – Midwesco What happened During rigging-up the work floor, the support legs were being lifted from the v-door, by winch using a tugger line. The Derrick man was guiding the tugger line with his left hand. As the support leg came off the v-door, it dropped slightly crushing his finger against the cat walk resulting in a fracture of the tip of his right hand ring finger. PDO Incident First Alert – Barik Oil Field Services What happened After completing rigging down the top section of the scaffolding, a technician began removing a plank (1m section of scaffolding grating). When he tried to push the plank up with the hammer and his hands, he slipped and fell on to the xmas tree and hit his hand against the xmas tree suffering a fracture to his upper right arm. PDO Incident First Alert – Midwesco What happened While trying to fill the hoist diesel tank from a diesel drum on a forklift forks the mechanic climbed over the barrier standing with one foot on the forks and the other on the hoist. 25. PDO Incident First Alert – MEDCO (Gulf Drilling) What happened A crane had just lowered a Top Drive System down onto the ground. An engineer attempted to disconnect the Top Drives lifting loop (bail) from the cranes hook but it was stuck. PDO Incident First Alert – Arabian Industries What happened A pickup truck travelling from Nimr to Marmul and the 3 ton canter truck travelling in the opposite direction were involved in a head on collision 3km outside of Marmul. The pickup driver suffered multiple injuries and the other two (pickup passenger & canter driver) received first aid treatment. Mr. Musleh asks the questions of can it happen to you? 29. PDO Incident First Alert – Shaleem What happened The Forklift Truck (FLT) Operator and Floor man were removing a pipe from the catwalk. As the FLT operator lifted the pipe, the Floor man tried to stop the pipe from rolling with his foot; the pipe slipped off the forks and dropped 60cm hitting his foot fracturing his left ankle. 30. PDO Incident First Alert – Schlumberger What happened A logging engineer was feeding a slack cable into a clamp on a logging unit. The clamp however failed which caused the cable under 5000lbs of tension to slip through the clamp pulling his fingers into the mechanism and severed off three of his fingers. The driller tried to stop it and in doing so his hand was crushed between the spool and an upright part of the rig structure, fracturing two fingers. 32. PDO Incident First Alert – Baker Hughes What happened Whilst filling a liner hanger with polymer the engineer loosened the locking screws on the junk cover, lifted it and then re-locked it. PDO Incident First Alert – Thomssen Open valve gate What happened A mechanic was testing the operation of a hydraulic valve and was repositioning the valve gate with his hand inside the valve mechanism.  The control room operator with whom he had been coordinating by radio (to open and close the valve) accidently closed it amputating the mechanics finger tip. On reaching the radio room front door he turned and walked backwards up the steps into the cabin. As his colleague began to climb the steps the electrician lost his balance and fell over inside the cabin with the safe landing on his left hand crushing his ring finger. PDO Incident First Alert – Weatherford Outside of the valve What happened A rig crew were mixing cement in the mixing unit but found not much dry cement was flowing in via the cement suction line. They opened the line and a supervisor stuck his hand in to check the hydraulic valve was working. PDO Incident First Alert – Weatherford BOP What happened A derrick man signaled a crane operator with his right hand to lower a part of the blow out preventer down on to a fixing.  His left hand was resting on the bolts that the blow out preventer would drop onto and so as it was lowered it did so onto his hand crushing it against a studded bolt and fracturing his finger. It reacted violently with something in the drain which then lead to an explosion spraying the plumber with the acid causing severe chemical burns. The driller thinking the travelling block swivel was stuck tried to free it by hand and as it came free it rotated trapping his thumb against the body of traveling block . PDO Incident First Alert – Abraj Energy What happened A Drilling Superintendent lost control of his vehicle while taking a curve in the road, the vehicle has rolled over onto its right side, as a result he sustained injuries to his left hand and fingers. PDO Incident First Alert – IPC What happened The deceased stood between two Frac tanks and reached under one of the tanks to pull on the locking handle of a pin connection between truck and tank (5th wheel). A crane was in place to hold the tanks weight after the truck had driven out from under the tank. The crane boom was positioned slightly wrong and as the crane took the weight the tank swung sideward's crushing the deceased between the 2 tanks. He died shortly afterwards. He called the forklift truck to help push them back in line. To protect the casings he held two pieces of wood between the forks and casings but left his finger in between them.
