Background: admitted yesterday for atrial fibrillation to SCU, had not been taking her Coumadin for a few weeks. History of HTN, cardiomyopathy recently diagnosed with EF 46%. Smoked 1 PPD x 35 years. Medication includes Lisinopril 20mg po daily, Metoprolol 25 mg po bid, Lasix 20 mg po and Diltiazem 120mg daily. She has been alert and oriented all day with BP 130/80, P-110- R 20 Time: 4:30 p.m. Assessment: Recommendation:
