Goal #2 • By 2017, increase screening rates of selected preventive health screenings by 5%. • Screenable Cancers: Breast, Cervical, Colorectal • Heart Disease ABC’s: Aspirin, Blood Pressure, Cholesterol, Smoking • Diabetes: ABC’s: A1C • Obesity and Falls • Promote Chronic Disease Self Management
