Discipline Faculty (in this example, Perio Faculty): Swipe card to Approve Completed D9450.2 step and pop up grade card 3. Enter Professionalism grade for the session Grade Card 4. Enter Grade for D9450.2 step 5. Discipline Faculty (in this example, Perio Faculty): Swipe card to submit grades for Completed D9450.2 step

11. 10 After each discipline approves their respective D9450.x step, the student can begin adding procedures from the patient’s signed and approved, written Treatment Plan. The process is the same as that described above: • Open the patient’s History, then click Add Tx • Open the Codes List, select a Category, navigate down the “tree” to find the procedure code, highlight the code then click Accept • Enter the appropriate Phase and Sequence number for the procedure • Click Add to add the procedure as a Planned Treatment • Repeat sequence for each procedure in the Treatment Plan • Open the Checkout Screen (double click box next to patient’s name in the Rolodex); TP Faculty swipes card to approve the Planned Treatments After the complete Treatment Plan has been entered, the TP Faculty can grade the D9450 parent step. All D9450.x steps completed All procedures from written TP added and approved as Planned Treatment History Double click to open D9450 parent step; change status to ‘C’ompleted; click ‘Accept’ Open the Checkout Screen (double click box next to patient’s name in the Rolo- dex); TP Faculty swipes card to approve the D9450 parent step; grade card pops up; enter grades for Professionalism and D9450.
