Brief history of organization, community to be served, and target population.
Kiamichi Family Medical Center, Inc., a 501(c)3 organization, received its notice of grant award as a new start community health center in June 2002 and began providing health care services in Battiest in northern McCurtain County on September 3, 2002. Dental and behavioral health services are currently provided through referral arrangements. The Center contracts with a local pharmacy to provide 340B drugs. Kiamichi Family Medical Center is deemed for malpractice coverage under the Federal Tort Claims Act. The new access point will be located in Idabel, in southern McCurtain County, to serve the area of McCurtain County that is not currently in Kiamichi Family Medical Center’s service area. The service area is a rural area of the state and is approximately 50 miles, more than an hour’s drive over mostly two-lane, narrow, winding roads through mountainous terrain, from a health service provider utilizing a sliding fee scale. Because of its high poverty Idabel and the nearby community of Garvin have been designated as a part of a federal Enterprise Community. Services will be targeted to the uninsured, Medicaid, and obstetric patients living in southern McCurtain County including the towns of Broken Bow, Garvin, Haworth, Idabel, Millerton, Valliant, and the rural areas. This rural area is federally-designated as a Medically Underserved Area, as well as a dental and behavioral health HPSA.
Major health care needs and barriers to care to be addressed.
The major health care needs of the targeted population are preventive and primary health care. There are high rates of cancer, diabetes, coronary heart disease, teen pregnancy, obesity, suicide, unintentional injury, and tuberculosis, and low birthweight rates. Barriers to care include high poverty levels, lack of insurance, lack of providers for obstetrical care, and the geographical location. Fifty-five percent of the population is living below 200% of the Federal Poverty Level and 27.54% is uninsured.

Providers totaling 2.5 FTEs will provide preventive and primary health care services at a clinic at 601 E. Washington in Idabel. Total FTEs will be 10.5. Dental patients will be referred to the Battiest site. Behavioral health will be provided on a referral basis. A local pharmacy will be contracted to provide 340B drugs. Total users at the end of the 3 year project period will be 2,600. Total encounters will be 8,400. The New Access Point will coordinate with Oklahoma Primary Care Association, Oklahoma State and McCurtain County Department of Health, OSU Extension Services, McCurtain Memorial Hospital, and Little Dixie Community Action Transit.
