15 yo male • fist injury • swelling and pain • Palpableemphysema
computerized tomography imaging • nondisplaced fracture on the medial wall • superior and inferior part of the globe
patient’s follow-up • pain with ocular movement • diplopia in lateral gaze • increase emphysema with maneuver of the valsalva
percutaneous aspiration-decompression • 21 gauge needle • under local anesthesia
complaints were decreased after the procedure • air was disappeared gradually in CT
Orbital emphysema • usually benign and limits itself • valve effect may cause visual impairment • this situation may cause ischemic optic neuropathy • decompression treatment is essential
conclusion Needle aspiration-decompression is a considerably effective and reliable technique in emergency situations
