Student is currently a Musical Theater major and wants to switch to Music Education with a Voice sub-plan and Voice Performance as a secondary major. • College = A0 • Degree = BFA • See example of how this would look in SIS below. • Student’s new status: • College = A0 • Degree = BMU • See example of how this would look in SIS below. • Since the degree-type is changing, use a PROGRAM CHANGE form to request the change in UAccess.

If you need to add a secondary program, like in the example below, use the PROGRAM ADD form.
If the student is adding a second major, that belongs to a different college, you use a PLAN CHANGE ONLY form, as long as the second major is in the same degree family.
