Navigate to Patient Safety ERShttps://ers.wusm.wustl.edu • General Event Information • Person Affected • Required Fields:Last Name, First Name, Gender, Date of Birth • Event Details • Required Fields:Event Date, Site, Department, Division, Entered Date • Specific Event Details • Followup List • Required Fields:Followup Date, Followup Type, Followup By • Event Summary

  3. On the General Event Information Form, select the Classification of Person Affected and General Event Type

  4. On the Specific Event Details Form, select the Specific Event Type; click Add/Remove to select Contributing Factors and Actions Taken

  5. Select all Contributing Factors that Apply

  6. Select all Actions Taken that Apply

  7. Select the Reported Event Severity that Applies

  8. Select all answers that address where in the process the event first occurred

  9. Enter a Brief Factual Description of the event to complete the Specific Event Details Form

  10. Review the Event Summary Form and then submit the event
