Background • Formed in 2002 as a legally constituted 501 (c ) (3) not-for-profit organization by the governing body of the State of Maryland. • 100% volunteer-operated organization, inc • Established by several community entities, concerned religious leaders and institutions of higher education. • Committed to addressing the socio-economic and health disparities that exist in the Southwest Baltimore area. • March 2013- DHMH announced an opportunity for community-based organizations to apply for a Minority Health Disparities Reduction Demonstration Grant for fiscal year 2014 (Baltimore’s focus was Cardiovascular Disease) • April 2013-Faith Center completed the grant application • June 2013-$75,000 Grant Award was announced
Cardiovascular Disease (CVD) & Baltimore City • Health disparities exist for every major indicator of health in Baltimore City. • Baltimore City Health Department, the city of Baltimore received a “D” grade for health indicators and health outcomes. • CVD was the number one cause of preventable death in the city amongst residents, claiming approximately 2,000 lives each year in the city. • Heart disease is a leading cause of the 20-year life expectancy gap among neighborhoods in Baltimore City. • Baltimore residents die more than six years before other Maryland residents • a significant portion of this difference can be attributed to heart disease. • Baltimore’s death rate for heart disease is approximately 30% higher than the state and national rates while stroke and diabetes also reveal higher death rates in the city
Program Objectives and Performance Measures • Objective 1:Toenroll 500 minority men and women in the Healthy Heartbeats program. • Record # of clients that show improvement in hypertension-related risk factors • Record # of clients that keep their appointments as referred to by CHWs. • Objective 2: To establish/maintain a local Minority Adult Cardiovascular Disease Reduction Coalition Group by the end of FY 2014. • Convene at least 4 (quarterly) coalition meetings by the end of FY 2014. • Recruit coalition members in all nine geographical areas of Baltimore City. • Objective 3: To develop new partnerships between the Healthy Heartbeats Program and community groups, healthcare and service providers and other stakeholders. • Develop at least 5 new partnerships each year. • Objective 4: To increase the education, awareness, and improved health behavior of racial/ethnic groups regarding minority adult cardiovascular disease. • Conduct/participate in monthly health-related events in community settings • Distribute at least 2000 health education materials; 2500 minorities reached
Lessons Learned • Once you enroll HHB participants at an event, follow-up and start the program immediately • Incentivize participation • Shorten HHB curriculum from 6 to 4 weeks or less • Leverage American Heart Association’s online tools such as Heart360 • Standardize CHW training • Payroll is not easy! Get someone to do it for you. • Be intentional about targeting minority populations (African American, Hispanic, Asian)
