  2. The Good Health Sub group has • Met 2 times since our last update • We now have our action plan. • It is called ‘The Big Health Plan’. • There are copies on your tables.

  3. The Big Health Plan • Healthy weight • Annual Health Checks and Black books • PCT Self Assessment (Big health check up)

  4. Healthy Weight • The PCT has started work on it’s healthy weight for Bromley plan • Maz and Stella are helping the PCT to include people with Learning Disabilities. • Maz, Stella and Rob are going to put together a twin plan especially for people with LD. The work will start soon. It will fit with the PCT plan.

  5. Annual Health Checks - so far • 239 checks were done last year • We have agreed 28 extra names since then • Numbers for this year due end of march • GP practices and Community LD nurses have been working hard to make sure the right people are on the list. • The PCT is thinking about how to get a health check if your doctor is not in the scheme.

  6. Annual health checks - to do • More GP’s need to join the scheme, but we do not know how long the scheme will run for. • Someone needs to have overall lead for checks and black books (The PCT and Oxleas are talking about how this can happen, but there is no extra money) • Community LD nurses need to roll out Health facilitator training more widely as the 1st training went well. Health facilitators are the people who will help you with your book.

Black Books • 363 books have been given out so far • 30 extra books have been given out by LD nurses as the lists are updated and we offer them directly to some people • 10 young people have had transition health plans, with 5 more in progress. • You may now get a black book from the LD nurses if your GP isn’t in the scheme.

  8. Black Books - to do • Once you’ve got one, you may need help to fill it out. • LD nurses can help you fill out the book if you see a lot of doctors. • Checking who has been given a book, find out how well the books are filled out, and how they are used. • Collect info from the books to plan service improvements • “The important thing is you’ve got to use it, and take it everywhere you go” - Laura
