  1. Student Responsibilities Please use a scale of one to five, five being the best, to rate your performance in the following areas. 1. I follow the four steps to homework success. (1. Bring it home 2. Complete it to the best of my ability 3. Bring it back to school 4. Turn it in on time) 2. I listen while my teacher is speaking 3. I listen while my classmates are speaking

  2. 4. I follow written directions on the board and on homework assignments. 5. I follow directions that are given aloud by my teacher. 6. I keep my work organized in my desk and my binders. 7. I remember to raise my hand when I want to share my thoughts and ideas or ask a question 8. I am able to work independently on my assignments in class.

  3. 9. I work well with others in a group activity. 10. I stay on task in class. 11. I do not disturb others in class. 12. I participate in class discussions.
