Getting Ready Preparing while incarcerated Share and Prepare • Advanced notice of release with basic demographic information is shared for PRCS and 1170H population • Through Multiple Disciplinary Teams (MDT’s), develop preliminary plans for each realigned supervisee • Initial engagement while person is incarcerated at the local level
Coming Home Arriving to San Mateo County Report and Referral • Transition from in to out-of-custody supervision • Orientation and warm hand-off to Probation/Service Connect • Alcohol and Drug Assessment and Referral Process • “Safety Net Bundle” • Engagement and Case Management • Supportive Services • Employment Services • Other Referral Services

Opportunities • Employment Providing on-the-job training opportunities, reducing reliance on entitlement programs • Family Reunification Encouraging parent engagement to prevent cycle of generational incarceration • 1115 Waiver Feds reimburse up to 50% of costs for health services IF on Medi-Cal or local coverage AND the services with Health System • Allowing Expertise Divisions/Departments do what they do best • Changing the Perception Reshape the paradigm in which government is perceived
