Previous referral link • Staff identify an individual with fire risk, obtain consent and access the Home Fire Safety Visit referral form via Staffnet • The staff member completes the referral form and saves in the hard drive • The referral form forwarded to Fire & Rescue logging in via GG-UHB.Fireassessment@nhs.net • Fire & Rescue open the incoming referral form and transfer the information to inbox of the local fire station (linked to the post code) • Local fire station manage the visit according to the risk score This process is being phased out All referrals from 1st November 2013 to be sent via the NEW online access The following slidesoutline the amended instruction

Step 3 Click the red text - one of our partners referring someone for a home fire safety visit?
Step 4 1st time user - please register a new account Returning user - please login as directed
Step 5 The referral form Complete the form and once complete click the blue SUBMIT button at the end. The form will automatically go to the nearest fire station to the client’s postcode
