Purpose of Treatment Plan • A = assessment • P = treatment plan • I = plan implemented • E = progress notes, discharge note • Purpose of plan is to identify services needed to achieve client goals and objectives
2 Types of Treatment Plans • Comprehensive plan • Team/agency • Discipline-specific plan • TR
Comprehensive Plan • Required by accreditation/regulation bodies • Includes entire scope of services that will be provided to client • Input from all disciplines & integrated into 1 document • Used to direct & evaluate services • Reviewed during treatment plan meetings • Could include area TR works on alone, or TR may work with others on a common goal
Discipline-Specific Plan • Required by TR standards • Compliments comprehensive plan • Focus exclusively on what TR will do • Includes goal, outcomes, intervention which are within TR scope of practice
Planning Process • Assessment, assessment summary • Make sense of assessment data • Identify concerns/strengths • Prioritize • Identify goals • LTG = what’s expected by discharge or annual review • STG = briefer time e.g., 2 weeks • Objectives • Intervention strategies • Program names, frequency, duration, facilitation styles • Evaluation schedule
