PGF follow up procedure • 1. What’s required • Contract specifications -Follow up each brief, full & facilitation interventions • -1, 3, 6, 12 months after discharge • -Screen? 3, 6, 12 months if appropriate • -Self care including suicide check • -Calls range from 2 mins to 30 mins duration
2.What we offer: During Treatment Ring at 1, 3, 6, 12 months after onset of treatment or when requested Provide ongoing support during treatment Provide continuity of care – same counsellor calls each time Relationship established prior to discharge Makes participation more likely after discharge Allows for interim safety checks upon request by counsellors (ie suicide alert support) PGF follow up procedure
After the completion of treatment 1 month follow-up can be helpful if clients have dropped out of treatment due to a slip Some clients re-engage with counselling when contacted via f/up – to date this is around 15% Safety check done – eating, sleeping, other interests Contract next call Canadian Gambling Screen if appropriate PGF follow up procedure
3 month follow-up CPGI Safety check done 15%have re-engaged with counselling 5% wish to be left alone or cannot be reached – most are the latter – to date only 1 person has asked not to be called again 80% re-contract for next call PGF follow up procedure
3. How we do it: At induction, counsellor asks client to fill out and sign this form PGF Follow-up Agreement Date:…………………………… Client Name:………………………………………ID No:………………………….. As part of our service we will provide ongoing support to your treatment @ 1, 3, 6 and 12 monthly intervals. We will follow up the same schedule once you have stopped seeing your counsellor. This also helps us in planning better services. I agree to receiving telephone calls from PGF as ongoing support for me during and at the end of my treatment to discuss my progress. Information from follow up calls may be passed onto your counsellor.
