How do I get there?(What do I need to help me reach my goals?) • IEP Annual Goals • Course of Study • Coordinated Set of Activities • Identify Agency Linkages
What is a Coordinated Set of Activities? • Activities, based on student needs, that will move the student toward accomplishment of his/her post secondary goals • Persons responsible for performing the activity are identified, as well as when it will be completed
Think of It As An ACTION PLAN! WHAT? WHO? BY WHEN? What activities Who is When will need to be responsible? they be accomplished? done?
A Coordinated Set of Activities Includes: • Instruction • Related Service • Community Experiences • Employment & other post-school objectives And when appropriate: • Daily Living Skills • Functional Vocational Evaluation
Let’s Take a Look at an Example Henry Forbes, Age 15 – 10th grade Post-Secondary Goal for Employment: After graduation, Henry will obtain employment in the graphic arts field. Post Secondary Goal for Education: After graduation, Henry will attend a 2 year college and major in graphic arts. www.tslp.org
All About Henry: • Strengths: • good hygiene & grooming • good listening & verbal skills • creative and artistic • good people person • Preferences & Interests: • likes art • likes working on the computer www.tslp.org • Info. from ESTR-J, teacher interview and Post-Secondary checklist: • feels good about self • completed intake for DARS • schedules own appointments • treats minor medical problems • corresponds in written form • does own calculations with calculator
Indicator 13 Are there annual IEP goals related to the student’s transition service needs? • Transition services that relate to post-secondary goals • For each measurable post-secondary goal there should be an annual goal included in the IEP related to the student’s transition services needs.
