Heat Tolerance Testing in Athletics Lack of heat chamber? No exercise physiologist?
Heat Tolerance Testing in Athletics • Lack of literature on heat tolerance testing with athletes • Is heat tolerance testing feasible at the high school setting? • Other athletic settings?
Athletic Considerations to Thermal Tolerance & Return to Play • Physician clearance • Inter-Association Task Force on EHI, 2004 • Gradual return to exercise • Inter-Association Task Force on EHI, 2004 • Thermal tolerance testing • Can military perspective be applied?
Physician’s Clearance • Asymptomatic or cleared to exercise in the heat? • Does clinically normal = heat tolerant? • What is the athlete cleared to do?
Gradual Return to Exercise • Mild Moderate Strenuous • Climate controlled vs Heat • Heat chamber vs Outdoors • Phasing in of equipment?
Thermal Tolerance Testing • What can be done without an exercise physiologist and/or heat chamber? • Single tolerance test or progressive RTP? • Ingestible thermistor or rectal thermistor? • Treadmill Sport-Specific Activities • ATC Supervision • Phasing in of equipment?
Example 1 A collegiate football player suffers EHS during a pre-season practice. He is cooled properly and after resting for 1 week, lab results are normal. The ATC and team physician decide he can RTP. • Mild-moderate treadmill walking in controlled environment (helmet only) • Mild treadmill walking in heat (helmet only) • Mild treadmill walking in heat (helmet/shoulder pads) • Mild treadmill walking in heat (full FB uniform) • Moderate exercise in heat (full FB uniform) • Sport-specific skills in heat (full FB uniform) • Full participation Sweat rate, HR, and TGI would be monitored throughout progression
Example 2 A high school soccer player suffers EHS during a pre-season practice. He is cooled properly and is cleared by his family physician the following day. A treadmill and HR monitor are available. • 120-min Treadmill test in hot environment • Walking: 5km/h and 2% incline • Monitor HR and calculate sweat rate • Stop the test if the following occur: • Any signs/symptoms of heat illness present • Heart rate above 160 bpm • If asymptomatic, return to soccer?
