Step by Step Instructions on how to complete the GEN 1321 form Reasons to complete GEN1321: If you wish to apply for WIS Fraud System access; If you need to terminate access due to change of employment; If you have been revoked by the system and need to have your access restored; If you forgot your password, cannot log on and need to have your password reset; If you have not logged on in over 90 days; If you have changed your name; If any other significant changes have occurred.
Steps to Completing the GEN 1321 (WIS ACCESS) Section 1, page 1 Section 2, page 1 In “USER NAME” portion write your last and first name as well as MI if needed. If you are simply changing your name – check the “Check if name change” box and complete this portion with your new name, then beneath in the “FORMER NAME” portion write your old name. In the “ADDRESS,” “CITY,” “STATE,” and “ZIP CODE,” portions write your mailing address. In the “PHONE NUMBER” portion, if you have a direct phone number, use that phone number. If your phone message identifies you as the person to whom that line belongs, the staff assigning logon IDs can leave your password on your voice mail. This is extremely useful. Include “FAX NUMBER” and “E-MAIL ADDRESS.” • Check ONLY ONE of the boxes. You are either a brand new WIS user – Check “New Access” box. You need to terminate your or an employee’s access – Check “Terminate Access” box. Everything else – Check “Other” and explain, e.g. “Reset password,” “Restore user access.” • On the “Current Bureau/Unit/Index” line be sure to write your county as well as any other appropriate information, e.g. “Inyo Co. DHHS.” Do not complete any other portions of Section 1.

The exception is termination of user’s access. In that case, the supervisor must check box 4. The user’s supervisor must print his or her name legibly, sign and date in this section. • If you are a new user or terminating access for a user do not complete this section. It is unnecessary. For restoring access or password reset, indicate here if the user forgot password, if the user revoked her or himself and needs to be restored, etc.

FAQ’s Q. How long does it take to process? A. It can take anywhere from a week to two weeks to process your GEN1321. Because of the series of signatures from different parties you have to give a few days just in case staff members are not all in the office on that specific day. Also, while we await your original signature to be received in the mail. Q. Who do I contact once I find out I’m locked out? A. Please immediately send in a GEN1321 via fax or email to fraudsystemsaccess@dss.ca.gov to either request or submit your GEN1321. Q. Who will contact me to notify me that my password has been reset? A. Depending on the type of request Tumbleweed IEVS or WIS and or RACF will contact you to inform you that your password has been reset. (For example the RACF group will contact you for a password reset for WIS, Katharine Jordan for Tumbleweed WIS and IssacVallee for Tumbleweed IEVS. Q. How many user ids can I have? A. Depending on the system you are only supposed to have one user account for Tumbleweed per county. If you need additional user id’s please submit a GEN1321 Tumbleweed New User Access so an additional user can be prepared.
