Ultrasound and Nerve Stimulator guided nerve block treatment of spasticity practicum Figure 1 Pre-nerve block Figure 2 Post-Nerve block Overview: Spasticity is a common problem among individuals who have upper motor neuron disorders. Botulinum toxin injection is a common interventional procedure done for treatment of Spasticity. Nerve blocks are rarely done yet they have a major role in treatment of spasticity. Nerve blocks are particularly useful as botulinum toxin is expensive and limited by the number of units that can be injected at a given time, it can only be repeated every 3 months. Also, nerve blocks can be done in combination with botulinum toxin when botulinum toxin is not sufficient to control the spasticity. Phenol is the medication used for nerve blocks for spasticity. It works by demyelinating the peripheral nerve thereby decreasing spasticity in the muscle groups supplied by that nerve. Nerve blocks are done better under ultrasound guidance as the nerves and blood vessels can be visualized accurately thereby avoiding the chance of injecting into unwanted structures. The accuracy is also improved with Nerve Stimulator, which will cause visible contractions of the muscles supplied by the nerve that is being stimulated.
