  3. Common Findings • Hazards not identified • Lack of effective supervision • Using the wrong tools • Not following rules/procedures • Poor communication (Language barrier ?) • Behaviour & attitude • Lack of concentration • Time pressure (hurry to finish the job)

  4. LTI 1: Broken Finger1-Jan • What happened?: • Finger trapped between bucket and rock breaker when • offloading the bucket from the trailer. • Result: • Broken finger • 3 weeks lost work • Learning: • Wrong hand placement • Poor position • Poor communication • Could this be avoided?

  5. LTI 2: Broken Finger3-Jan • What happened • Pipe spinner (heavy tool hanging from a cable) swung back and trapped the floorman’s finger. • Result: • Broken finger • 3 weeks lost work • Learning: • Improper position • Wrong hand placement • Absentminded/Unfocused • Could this be avoided?

  6. LTI:3 Broken Arm 24-Jan • What happened?: • A rigger placed his arm into the recess of the crane • outrigger as it was being retracted. His arm was crushed. • Result: • Broken arm • 6 weeks lost work • Learning: • Wrong position (rigger) • Wrong position (operator) • Poor communication • Poor supervision • Could this be avoided?

  7. LTI:4 Ruptured spleen 6-Feb • What happened?: • A food delivery truck travelling to Marmul. The driver fell • asleep. The truck left the road, rolled and crashed. The • driver was stuck for 2 hrs in the badly crushed cabin. • Result: • Ruptured spleen(can be fatal) • 2 months lost work • Lost truck & load • Learning: • Journey Plan not followed • Driver not authorised • Poor Safety attitude • Could this be avoided?

  8. LTI 5: Broken Leg 23-Feb • What happened?: • Rigger climbed on the cabin to release the lifting slings. • He did not use a ladder. When coming down his foot • slipped and he fell. • Result: • Broken leg • 6 weeks lost work • Learning: • Hazard not identified • Not using the right tools • Poor supervision • Could this be avoided?

  9. LTI 6: Back injury9-Mar • What happened?: • Helper came out of the truck looking forward instead of • backward. He lost his his footing on the first step of the • ladder and fell down hitting his back. • Result: • Back injury • 2 weeks lost work • Learning: • Rule not followed • Wrong way of disembarking a truck • Could this be avoided?

  10. LTI 7&8: Neck injury &13-Marbroken arm • What happened?: • Canter truck delivering most-urgent chemicals: night • driving, very bad road due to rain, long driving hours. The • truck rolled over after hitting a bad spot on the road. • Result: • Driver: neck injury • Helper: broken arm • 2&6 weeks lost work • Learning: • Poor planning/JM • 2 drivers on difficult trips • Contact TLM for urgent deliveries • Could this be avoided?

  11. LTI 9: Broken finger25-Mar • What happened?: • Thread protector stuck on a drill-pipe joint. Rigger takes • hammer to free it. After two blows his colleague placed • his hand on the protector to remove it just as the the third • hammer blow was given. • Result: • Broken finger • 2 weeks lost work • Learning: • Wrong tools. • Poor communication. • Hazard not identified • Could this be avoided?

  12. LTI 10: RTA Fatality3-Apr • What happened?: • Truck delivering line pipe to Fahud. Driver probably fell • asleep. Went straight of off the road, rolled-over and • jack-knifed. Cabin completely crushed. • Result: • Driver dead • Family lost their father • Lost truck • Learning: • JM plan NOT followed • STOP when tired • Could this be avoided?

  13. LTI 11: Lost Finger 5-Apr • What happened?: • A helper was disconnecting a trailer from an oil-field truck • winch cable. He signaled the driver to slacken-off the • winch. Winch operator misunderstood and pulled-in • instead. • Result: • 2 lost fingers • 4 weeks lost work • Learning: • Wrong hand placement • Poor communication • Could this be avoided?

  14. LTI 12: Elbow Dislocation 7-Apr • What happened?: • Crane operator drove too close to the edge of an • excavation. Crane crashed down into the 3 m deep pit. • Operator did have his seatbelt on. • Result: • Dislocated elbow • 4-6 weeks lost work • Major crane damage • Learning: • Hazards not identified • In a hurry • Could this be avoided?

LTI 13: Crushed Finger20-Apr • What happened?: • A staff member was adjusting her shoe strap whilst walking. • She lost her balance and caught herself by placing her • hand against the doorframe. Her finger got trapped by the • spring loaded closing door. • Result: • Crushed finger • 2 weeks lost work • Learning: • Do not try 2 activitiesat the same time • Could this be avoided?

  16. LTI 14: Broken Finger28-Apr • What happened?: • A floorman was instructed to use the rig tong to break a pipe • connection. Instead, a colleague brought a big hammer and • started to hit at the assembly. The hammer slipped and fell • down onto the floorman’s hand. • Result: • Broken finger • 3 weeks lost work • Learning: • Not following instructions. • Using wrong tools. • Wrong position. • Rush to finish the task. • Could this be avoided?

  17. LTI 15: Broken Hip29-Apr • What happened?: • A technician placed a ladder against a storage rack and • climbed up to look for a flange. When coming down the • ladder slipped and collapsed. He jumped off but knocked • over a heavy crossover flange which in turn landed on his • hip. • Result: • Broken hip • 3 weeks lost work • Learning: • Hazard not identified • Procedures not followed • Never go up un-secured • ladders • Could this be avoided?

  18. LTI 16: Broken Arm 2-May • What happened?: • A technician was drilling a steel bar using a pedestal • workshop drill. The bar was clamped but slipped out of the • jig. He tried to grab the steel bar with his hands. His glove • caught in the drill and hands and arm were pulled into it. • Result: • Broken arm/open fracture • 2 months lost work • Learning: • Inadequate supervision • No list of authorised equipment users • Could this be avoided?
