Root Cause Analysis • Root cause analysis is a method of problem solving where the root cause is found after several iterations of asking why a particular phenomenon exists • A root cause is an underlying issue that activates the cause-and-effect response that leads to the visible problem • After the cause is found, continuous improvement methods are used to eliminate/mitigate the root cause @EducateIN @ntwilliamson @GinaGRomano
