  3. Ground Rules • Create before team begins to work on project • Establishes the foundation • How do you want to work together as a team? • Possible Examples: • Don’t interrupt • Be open-minded • Listen and respect each other’s opinions / ideas

  4. Accountability • How are you going to hold each other accountable? • Have a plan in place

  5. Agenda • Preparing for the meeting- plan the agenda Define the intent of the meeting and roles • Establish time to cover agenda times • Define actions to be taken in meeting and responsible parties • Share expectations with team members

  6. Delegation of Roles in the Meeting • Four essential roles • Meeting Leader • Time Keeper • Recorder • Facilitator

Communication Plan • How are you going to communicate your accomplishments throughout your Practice / organization? • People feel included and in the loop • Possible Tools / Methods: • Weekly summary of team meetings • Team meeting notes • Communication Board • Organization Newsletter • Weekly / Monthly Staff & Provider meetings
