  1. Functions, duties and Responsibilities of a Teacher-Registrar • The teacher registrar is in charge with the sole responsibility of keeping pupils’ records, particularly academic records.

  4. FORM 1 (SCHOOL REGISTER) An attendance register is a book in which the presence or absence of pupils in a school is recorded on a daily basis. It is a statutory record that must be kept by every school. This record is kept on individual class basis. The class teacher is the custodian of this record. It is helpful in identifying sick pupils, truants, absentees and pupils who attend school regularly.

  5. FORM 1 • Entries on attendance in Form 1 should be complete and should reconcile with the teacher’s narrative remarks in Forms 2, 137, 138 and other forms. • Record of textbooks released to pupils should be accomplished by the teacher.

  6. Form 1 should reflect the actual absences (non-attendance) of pupils. It contains permanent data, hence should be accomplished in ballpen (not pencil).

  7. CLASS RECORD • All grades should be computed in the class record. • School heads should look into the accuracy of grade computation particularly the weight for each criterion in consonance with the following DepEd Orders:

  8. DepEd Order No. 33, S. 2004 (Implementing Guidelines on the Performance-Based Grading System for SY 2004-2005) • DepEd Order No. 39, S. 2004 (Corrigendum to DepEd Order No. 33, S. 2004)

  9. CLASS RECORD • DepEd Order No. 26, S. 2005 (Character Education as a Separate Subject of MAKABAYAN for the Elementary Level) • DepEd Order No. 31, S. 2012 (Policy Guidelines on the Implementation of Grades 1 to 10 of the K to 12 Basic Education Curriculum

  10. CLASS RECORD • The final rating shall be determined by averaging the ratings for the four quarters. • From the class record, grades are transferred to the grading sheets.

  11. GRADING SHEET • Grades in the class record should reconcile with the entries in the grading sheet. • Avoid erasures in all forms so as not to cast doubts.

  12. FORM 137 Pupil’s cumulative record folder is a storehouse of information on pupil’s academic performance. It reflects continuous assessment on pupil’s educational or academic progress.

  13. FORM 137 • Form 137 should be completely filled out by teachers concerned. • Erasures should be avoided since it is a permanent record of the pupils. In case there are erasures, they should be accompanied by initial of concerned teacher.

  14. To compute the rating for Makabayan, follow the procedure below: (Enclosure No. 2-A to DepEd Order No. 26, S. 2005: • For Grades II-III, add the ratings for Sibika at Kultura and Character Education and divide by 2.

  15. For Grades IV-VI, add the ratings for HEKASI, MSEP, EPP and Character Education then divide by 4. • To get the General Average, add all the grades of the 4 core subjects plus Makabayan and divide by 5.

  16. In elementary, all entries in Form 137 should also appear in the Form 138 and in Form 18-E. In Form 18-E, the final rating is rounded to the nearest whole number (Page 3 of Enclosure No. 1 to DepEd Order No.33, S. 2004).

  17. The components of Makabayan with their corresponding ratings should appear in Forms 137 and 138. However, final ratings in 18-E (Elementary) should be rounded to the nearest whole number

General Weighted Average (GWA) in Forms 137 and 138 are expressed up to 2 decimal places. • Name of adviser with signature should appear after each grade level completed in Form 137(starting this SY 2012-2013). • Summer term is part of the preceding (past) school year and should not be counted as additional year as reflected in the total number of years in school to date.

  19. FORM 138 (REPORT CARD) It keeps data on pupils’ academic performance in periodic basis. It assists teachers and parents in monitoring pupils’ academic progress. It is a compliment to the student’s cumulative record

FORM 138 (REPORT CARD) • For parents’ tracking of their children’s performance, regular release of Form 138 should be observed. • Attendance record of pupils in Form 138 should reconcile with those in other forms. • In CE, specific indicators (found in pages 6-9 of Enclosure No.1 to DepEd Order No. 33, S. 2004) shall be used in rating the 14 traits (number may vary) of pupils enumerated in Form138.

  21. The lowest failing grade that may appear in the report card is set at 65 percent (Item 1, DepEd Order No.33, S. 2004). • Failing marks in Form 138 should not be written in red ink.

  22. As indicated, Certificate of Transfer should only be signed in case of transfer. • Entries in Form 138 should come from Form 137. • Under remarks, passed/failed should be reflected (Grade I under K to 12 Curriculum)

  23. FORM 18-E • Check on the Curriculum entry in Form 18-E • BEC Basic Education Curriculum • K to 12 Basic Education Curriculum for Grades 1 and 7 (this SY 2012-2013)

  24. All dates in 18-E shall take the date of the close of the school year. For this SY 2012-2013, all dates in 18-E is March 22, 2013.

  25. Only 1 final copy of Form 18-E should be submitted to the Division Office. • Teacher-advisers are advised to make use of the computerized DepEd Forms which are readily available.

  26. As required in the 18-E, subject teachers should affix their signatures in spaces at the bottom of the columns headed by the subject through. • As per instruction at the back of 18-E Form, Action Taken should use the word promoted (abbreviated to Prom.) or retained (abbreviated to Ret.) for all grade levels.

  27. DIPLOMA • Writing of name in the diploma should follow the First-Middle Initial-Surname order. • Date of diplomas issued to graduates should bear the actual date of graduation.

  28. All entries particularly the pupil’s name should be checked and verified correct by the School Checking Committee prior to signing of School Heads, Public Schools District Supervisors and Schools Division Superintendent.

  29. DepEdOrder No. 31, S. 2012 (Policy Guidelines on the Implementation of Grades 1 to 10 of the K to 12 Basic Education Curriculum

  30. For Grade I and Grade 7, the final grade for each learning area shall be reported as the average of the four quarterly ratings in terms of the level of proficiency.

  31. Level of Proficiency Equivalent Numerical Value • Beginning (B) 74% and below • Developing (D) 75-79% • Approaching Proficiency (AP) 80-84% • Proficient (P) 85-89% • Advanced (A) 90%

  32. The general average shall be expressed in terms of levels of proficiency with the numerical equivalent in parenthesis. • Promotion and retention of students shall be by subject.

  33. Honor students shall be drawn from among those who performed at the Advanced Level. Subsequent guidelines shall be issued as basis for ranking of honors (Enclosure No. 1 to DepEd Order No. 31, s. 2012).

  34. DepED ORDER No. 26 s. 2005 Character Education as a Separate Subject of Makabayan in the Elementary Since SY 2005-2006, Character Education has been taught as a subject of Makabayan in the elementary level curriculum

  35. Character Education shall be allotted 30 minutes daily for Grades I-III and 20 minutes for Grades IV-VI using Filipino as medium of instruction (Enclosure No.1) This period is strictly to be used for this purpose only and not on routinary matters in the classroom.

  36. A separate composite rating for character education shall be provided before obtaining the over-all rating in Makabayan. • Teachers handling subjects in Grade IV, V and VI shall give each pupil the rating for behavior observation and the average shall be obtained.

  37. In computing the General Average of a pupil, add the final ratings of the five learning areas and the sum divide by 5.

  38. For Grades I-III, Character Education may be at the beginning of the class period since the learning areas are handled by the same teacher. • For Grades IV-VI, Character Education may not be necessarily be at the beginning of the class period. The class adviser or other teacher may be assigned to handle it.

  39. Rating for Character Education A. Test Results and Other Performance Outputs 60% • Periodical test score 82 x .25 = 20.5 • Unit test 82 x .15 = 12.3 • Class participation 82 x .1 = 8.2 • Assignments 82 x .05 = 4.1 • Projects 82 x .05 = 4.1 60% 49.2

  40. B. Observation Results (Character Traits) 40% The class adviser and the other teachers handling the class shall give the rating for behavior observations. Behavior Observation has a weight of 40% in the overall rating for Character Education

  41. Pupil has an average of 84.75 x.40 or 33.9 • Example for Pupil A

  42. Therefore, the grade for test results and other measures of achievement which is 49.2 shall be added to the grade for observation results, which is 33.9. Total will be 83.1 or 83. • This becomes the final grade of Pupil A in Character Education.

  43. Reminders: • When you release Form 137 and B-certificate make sure that these are duly authenticated, signed by the school head or school registrar. • Submit list of Examiners for NCAE Y3, NAT Y4, NAT 3 and NAT 6 (see attached format) on the dates specified therein: • First Week of January for NAT 3, 6, and Y4 • Last week of July for NCAE Y3 (List of Examinees will not be submitted to the Division Office, only the total enrolment)

  44. Philippine Educational Placement Test (PEPT) • Pursuant to DepED Orders and Deped Memoranda, the Department of Education (DepED) through National Education Testing and Research Center (NETRC) administers the annual Philippine Educational Placement Test (PEPT) nationwide by school division. • It’s designed to help over-age pupils to catch up to where they should be.

  45. The Division Testing Coordinator (DTC) shall get from the school heads/School Testing Coordinators (elementary and secondary levels) the Actual List of PEPT Registrants following this format:

  46. PEPT is conducted (annually) every 3rd Sunday of November. Registration is set one - two months before the examination day. Documentary Requirements: Form 137 (2 copies) Birth Certificate (2 copies) Two 1x1 pictures

  47. Touching Story The story goes that some time ago, a man punished his 5-year-old daughter for wasting a roll of gold wrapping paper. Money was tight and he became annoyed when the child tried to decorate a box to put under the Christmas tree. Nevertheless, the little girl brought the gift to her father the next morning and said, "This is for you, Daddy."

  48. The man was embarrassed by his earlier overreaction, but his anger flared again when he found out the box was empty. He yelled at her, stating, "Don't you know, when you give someone a present, there is supposed to be something inside? The little girl looked up at him with tears in her eyes and cried, "Oh, Daddy, it's not empty at all. I blew kisses into the box. They're all for you, Daddy."

  49. The father was crushed. He put his arms around his little girl, and he begged for her forgiveness. Only a short time later, an accident took the life of the child. It is also told that her father kept that gold box by his bed for many years and, whenever he was discouraged, he would take out an imaginary kiss and remember the love of the child who had put it there.
