Clinical presentations of EV71 • Infection of EV71 can cause hand-foot-and-mouth disease (HFMD), herpangina, aseptic meningitis, acute flaccid paralysis (AFP), brainstem encephalitis, and pulmonary edema. • Several death-associated outbreaks of EV71 infections occurred in Taiwan from 1998 to 2005.

  15. Appearance of intratypic recombination of EV71 • Recombination is a well-known phenomenon for enteroviruses. • The actual extent of recombination within circulating EV71 is not known.

Summary of EV71 recombination study • Intratypic recombination of EV71 among nature circulating isolates was found to be initiated in 2002 and became predominant in 2004 and 2005. • Recombinant points were located at the 3'-termini of 2A and 3C regions, suggesting that nonstructural genes recombined to produce new EV71 variants.

  24. Implications of EV71 study • The results indicated that standard typing may produce misleading results or even sequencing of multiple partial regions of EV71 will not always be sufficient. • It remains to be determined what role of the emergence of this new EV71 recombinant plays (e.g. clinical significance, vaccine selection).
