MISSION STATEMENT • Provide patients with a seamless multidisciplinary approach to care • Streamline the access to final diagnosis and treatment • Provide pertinent patient education • Provide on-going support to patients and their caregivers • Include patients and their goals in treatment plans • Update referring and other physicians quickly; keep involved in treatment plan

MESOTHELIOMA SPECIALTY CARE CENTERWHAT PATIENT SHOULD EXPECT MESOTHELIOMA NURSE COORDINATOR: • Calls patient to perform needs assessment; reviews details of clinic visit • Schedules all visits with multidisciplinary team for same day • Contacts appropriate support services team members to see patient • Checks insurance to address issues • Accumulates all necessary medical records • Sends new patient packet • Navigates patient through the day of the visit • Obtains consent, completed questionnaire for tissue bank (NMVB). • Provides patient education materials, reviews with patient • Ensures patient/family understand recommended treatment plan • Contacts referring and other physicians day of visit to give brief update • Contacts patient after visit to assess understanding of treatment plan, stress importance of next steps to take
MESOTHELIOMA SPECIALTY CARE CENTERWHAT PATIENTS SHOULD EXPECT FROM PHYSICIANS: • Review of medical records, scans at multidisciplinary tumor board prior to seeing patient • Discuss recommended treatment plan with patients; respects patient goals • Determine if any treatment can be performed closer to home at patient’s approval • Ensure patient/family understand recommended treatment plan • Contact, coordinate treatment plan with any other physicians involved • Have staff arrange for any procedures, testing, follow up required • Send detailed visit summary to referring and other physicians
MESOTHELIOMA SPECIALTY CARE CLINICWHAT PATIENTS SHOULD EXPECT OTHER MEMBERS OF MULTIDISCIPLINARY TEAM: • Provide patient with identified needed services such as pain control • Provide any further patient education materials • Ensure patient’s understanding/approval of recommended treatment plan • Provides contact information • Coordinate treatment plan with other health care team members as needed • Provide follow up to any treatment they have recommended
MESOTHELIOMA SPECIALTY CARE CENTERFUTURE GOALS • Continue to obtain patient feedback in order to improve service • Update patient education materials on a regular basis • Identify need, begin local phasing to national support group • Professional conference planned for September, 2012 • Event planned for Mesothelioma Awareness Day, September 26, 2012 • Increase local, regional, national awareness of the Hillman Cancer Center’s Mesothelioma Specialty Care Center • Continue research efforts • Continue to actively recruit patients for the NMVB
