Flexor Carpi Ulnaris Tendonopathy Accession # 105218 Erica Fields, DVM
Dakota Vincent • 7 yo FS Weimaraner • Developed ST swelling – left caudal antebrachium – 1 mo prior to presentation, after playing outside • rDVM aspirated as mesenchymal neoplasia
Radiographs • Chip fractures associated with the radial carpal bone • Soft tissue swelling proximal to accessory carpal bone • No evidence of bony lysis
Ultrasound • Allows better examination of soft tissue structures (muscle, tendon, ligament) • Good spatial resolution • Can assess blood flow • Biopsy guidance
Anatomy • Flexor carpi ulnaris • Two bellies (ulnar and humeral heads) • Becomes flat tendon, inserts on acc. carpal • Sits lateral and deep to SDF • Flexes and abducts carpus • Prevention of hyperextension • Palmar fibrocartilage • Flexor retinaculum • Palmar ligaments Kuan, et al, 2007 and Whitelock, 2001
Anatomy • Tendons in general • Collagen fibrils surround parallel rows of fibroblasts • Symmetrical crimp • Paratenon and epitenon contain blood supply • Sheathed when crossing joints • Injury: capillary hemorrhage, ischemia, necrosis, fluid accumulation, granulation tissue Denny, 2004
Strain Injury • Injury to musculotendonous unit • First through third degrees • Can be acute or due to chronic stresses (e.g. racing Greyhounds) • Acute treatment: rest, cold, immobilization (3 wks) • Rehabilitation needed later to maintain function • Surgical tx needed for ruptures Denny, 2004
FCU Injury • Usually seen in racing Greyhounds • Reported in 1 Weimaraner • Our surgeons report seeing it in other Weimaraners • Repetitive (or acute?) stress injury Kuan, et al. 2007
