2013 Application Checklist • Designate the focus area you are applying to and complete the organization data in Section 1 of the Common Grant Application. (Make sure that this has been signed by board president and executive director.) • 2. Describe the program/project in Section 2-6 on five single-sided pages or less in 12 font letters . Please describe exactly how the Impact 100 funds will be used and the measureable outcomes that are anticipated • 3. Financial Statements to be included for each fiscal year • Fiscal years to be included: • キCurrent year to date • キ2012 • キ2011 • キ2010 • Type of Financial Statements to be included: • キIncome Statement • キBalance Sheet • キCash Flow

Agency Governance • Board positions/committees and terms of service • External employment firm and position
2013 Application Checklist • 7. Agency Audit for 2012, 2011, and 2010 • If your agency does not do a full audit, other acceptable documents include: • Audit and opinion letter, or • Non-Audit review (review, compilation, or company prepared), or • Form 990 • 8. Signed Terms of Agreement form
Submission Guidelines • Grant Applications must be submitted in two PDF files as follows: • First PDF is entire common grant application including all eight sections, required financial statements, agency and project budgets and required non financial statements with the exception of the 501 letter as we already have that on file from your LOI. This PDF should be labeled agency name, 2013 grant application. • Second PDF should contain Agency Audits for past three years and signed terms of agreement. This PDF should be labeled agency name, 2013 audit documents. • Please review all PDF files prior to submission to make sure pages are legible and that pages are in standard portrait view.
Submission Guidelines • Grants can be submitted in one of two ways in 2013 • Option 1 • Submitted via email to administration@impact100.org • Deadline for email submissions is 11:59pm Monday March 18th • Option 2 • Submitted on flash drive and mailed or hand delivered to Impact 100 by noon on Monday March 18th. Please note, for mailing of flash drives postmark must be no later than Friday March 15th and flash drive should be received by Monday March 18th
