REMINDERBlood Consent • Surgical consent is only valid up to 72 hours after surgery • Single informed blood consent is sufficient for the course of therapy • Serial medical outpatients, a single informed consent is valid for 6 months
REMINDERBlood Clot • Blood clot collection tube correctly labeled
REMINDERBlood Clot • Blood clot specimens go directly to Blood Bank • Blood supplies are all centrally located • Blood clot can be drawn 7 days prior to surgery but only good for 3 days once patient is admitted
REVISED POLICIES Blood Bank ID Bands • The witness for the blood clot draw will be eliminated in the policy due to the initiation of : • Patient ID verification • Blood ID banding • Bar coding
REVISED POLICIESBL003 policy • 5 policies into one policy- Administration of Blood Products (Attachment A) • The read back verification is added in the policy • Vital signs will be documented after 15-30 mls of blood instead of 15 minutes
REVISED POLICIES • The form #905412 will be eliminated • Blood transfusion will be included in the Handoff Communication policy • Patient will not be transferred until 15-30 mls of blood transfused
REFORMATBlood Bag Tag • The 15 minutes vital signs will be changed to vital signs after 15-30 mls transfused except for NICU and Pediatrics will be following their specific policy • Respiration is also added to the vital signs
Blood Bag tag • Checklist is added to the blood bag tag to remind nurses to check and complete the documentation. • The time of transfusion is separated to follow the sequence of event in the blood administration process
RE-FORMATBLOOD ID BAND • Apply the smallest patient EZ ID label to the “R” band • The blood band is now ¾’’ wide to accommodate the patient ID label
RE-FORMATBLOOD TRANSPORT • The blood transport using the pneumatic tube system will now have yellow lining that indicates safety • The pneumatic tube system will only be used during night shift
RE-FORMATBLOOD TRANSPORT • The front of the tube says BLOOD and the back says RETURN TO BLOOD BANK
