Grade 6 Requirements for Enrollment Required for ALL INCOMING GRADE 6 STUDENTS OR students born after born AFTER January 1 1997: • Meningococcal containing vaccine (Menactra) • Tdap (Tetanus, diptheria, pertussis) • Physical examination
Middle School Athletic Participation Physicals Two part form Part A- Health History Questionnaire Part B- Medical Athletic Examination Form Available on website. Must be accompanied by Athletic Permission Slip and Health History Update Form if physical is older than 60 days.
Medications • Bring all medications to nurse in original labeled container. • Medication Administration Form filled out by BOTH the parent and the Physician. • No Medication will be dispensed without proper documentation. • BY N.J. LAW- School nurses are not permitted to give over the counter medications without a doctor’s prescription.
Medications (cont’d) • Health office DOES NOT supply over the counter medications. • Asthma Action Plans and Allergy Action Plans are required along with 2 wallet size photos per medication. • Reminder: Medication orders are good ONLY for the school year and must be renewed annually.
Illness in School A child sent home with fever must be fever free without fever reducing medication for 24 hrs before returning to school. Must be without vomiting OR diarrhea 24 hrs. before returning to school. Suspected Strep or Conjunctivitis requires a clearance note from a doctor to return to school.
Injuries • Students arriving to school with an injury, crutches, and or cast must see the nurse and have an doctor’s note. • After treatment a Cleared to Return toSchool/Physical Education/Sports note is required. • Students who report to school with an injury, crutches and or cast without a doctor’s note will not be allowed to remain in school.
Physical Education Excuses • All requests for physical education excuses MUST come to the nurse. Only the school nurse can grant excuses. • An excuse for more than 3 days requires a note from a doctor and must be presented to the school nurse.
Absences and Tardiness • Please call the Health Office BEFORE 8:10am to notify the school of an absence or tardy situation. • Absences/ tardiness will be excused with a doctor’s note, documentation of court appearance, death in family, or religious observance.
Medications at Home and Other Medical Conditions • Please provide the Health Office with names, dosages, frequencies of medications taken at home as well as updates and changes. • Please provide the office with information about any underlying medical conditions even if you think it may NOT be an issue at school. • This information provides missing puzzle pieces that help the school nurse better address your child’s needs at school.
