In one year, Billy will follow the morning routines independently without verbal guidance for 3 days in a row. (Morning routines includes getting off the bus, walking to the correct door, going up the steps and through the door, finding and opening locker, hanging up his coat, entering classroom, and sitting at his desk ready to start the academic portion of the day.) Responsible parties: classroom teacher, PT, and OT
In one year, Jason will move from his homeroom to science class using his power wheelchair within the allotted 3 minutes before the bell rings at least 3 days each week. Responsible parties: homeroom teacher, science teacher, and PT
In one year, Adam will walk in line to the lunchroom with his class and move through the food line without assistance every day of each week. Responsible parties: PT, OT, lunch room staff, and classroom teacher
In one year, Tom will be independent in toilet transfers and self-catheterization with adult supervision on 3 consecutive opportunities. Responsible parties: School nurse, PT, OT, and classroom assistant/associate
In one year, Carolyn will play on three pieces of playground equipment during recess over a 2 day period. Responsible parties: PT, PE teacher and playground supervisor
During community mobility with his class, Tom will use his power wheelchair to obtain and pay for 3 grocery items from a list provided by his teacher. Responsible parties: PT, OT, and classroom teacher (for successful living)
In one year, Sally will follow the teacher’s instructions to sit at her desk and open her text book within the same time period as the rest of her class.
In one year and with adult supervision, Debbie will stand up from her wheelchair and use her walker to walk in line with her class to the lunchroom 3 days in a row.
In one year, Sam will attend 5 school activities with his family or peers and demonstrate appropriate behavior. (School activities might include theatrical plays, sports game/events, school sponsored dances, etc.) Responsible parties: psychologist, homeroom teacher, PT, special education teacher, school counselor
In one year, using her manual wheelchair, Suzie will leave class 2 minutes before the bell, use the elevator, and be in her next class at her desk by the time the bell rings on 3 Mondays in a month.
In one year, during preschool, Lynn will use her walker and move between 3 centers and participate in the activity (at the center) for at least 5 minutes before moving to the next center.
