  3. Locations • Employee Health / Wellness Center • Located in the Medical Center • First Floor Gerlach Building • (Old Admitting Area) Across from Spotted Cow • Peoria IL 61603 • Monday - Friday 6:30am - 5:00pm • 309 655-2429

  4. Difference BetweenRandolph and Glen Oak Sites Randolph Office: • Staffed with physicians, nurses, techs, secretaries, receptionists and billers. • Provides services for SFMC and 700+ other companies. • Have x-ray facilities, procedure room and supplies to treat most work injuries.

  5. Glen Oak Office • Staffed with one registered nurse and an MOA • Worker’s compensation case manager • Nurse practitioner • Sees only medical center employees • Treats initial work comp injuries • No x-ray facilities • Follow up after injuries

  6. Immunizations • Required immunizations for healthcare environment: • Two doses of measles-if born after 1-1-57 • Flu vaccines available. • Hepatitis B series - if in patient care area have vaccination or sign waiver. Series of 3 shots and blood work two months after final shot • Chicken Pox – certain dept. require titer drawn (anyone working with kids, oncology, surgery) • TB testing – annually/new employee 1 step or 2 step • Mumps – requires doctors documentation or have titer drawn

  7. Work-Related Injuries • All work related injuries, including exposures, or anything that happens on the job that could or did result in an injury. • Must call: 1-888-627-5673 (OSFMG employees will fill out an occurrence report and fax or hand deliver it to their manager/supervisor). • The call center RN will ask about your injury, provide treatment recommendations, help schedule related appointments, and initiate the reporting process.

  8. EXPOSURE • Exposure to blood and/ or body fluids • Need to call the CALL Center. • Follow their directions—go to ED or Occupational Health. • You will be instructed on completing paperwork, blood drawn, etc. • All paperwork is on line. On home page of Intranet click on Occupational Health.

  9. Illnesses • You should not come to work if ill. Especially if vomiting, diarrhea, or fever in last 24 hrs. • If you do your supervisor may send you over for FFD (fit for duty) assessment. • We do not treat personal illnesses, we will refer you to PCP (primary care provider).

  10. RTW Clearance • A physician’s release or a note from Occupational Health is required after an employee has been off work for 3 or more consecutive work days. • Can call us if you are unsure who you will need to see.
