  5. What is the problem? • Quality of healthcare and social care has improved • Health has improved but not everyone has the same opportunity for health and wellbeing • 1.5 million people die each year through early death

  6. Some specifics • People with learning disabilities are • 58 times more likely to die before the age of 50 when compared to the general population • and 4 times more likely to have a preventable cause of death • Carers UK report - In Poor Health • Analysis of the Census shows that nearly 21% of carers providing over 50 hours of care say they are in poor health compared to nearly 11% of the non-carer population.

  7. What is the solution? • The challenge is greater than any one organisation can manage • Work across system and networks • Challenge to leadership • Challenge to achieving cultural change which people own

The overarching role for LSPs • The LSP consists of all the key players in a local area who deliver services. ‘LSPs will provide the forum for setting the strategic vision for an area, for capturing that vision in the Sustainable Community Strategy, and for agreeing priorities for improvement in the LAA’ (LGWP 2006) …..Delivering on this shared vision will require partnership working to be the day job through… • Jointly identifying priorities • Co-ordinating consultation activities • Co-ordinating alignment of resources • Collective performance management • The leadership role for local authorities (driving consensus on priorities, community representation and scrutiny);

Achieving cultural change • Recognise cultural capital influences how people make decisions • Be clear about the population target and how behaviour is affected by cultural factors • Use segmentation and profiling • Tailor and personalise policy interventions • Communicate and engage • Big change occurs over the long term
