Technical Specifications Change • Page 5 – Outreach Effort: Outreach activities are conducted by Health Home or care management subcontractor staff to engage (or re-engage) members in Health Homes. • Attempts to contact members in person, via phone/text, or with individualized mailings/emails are included in the counts of outreach effort. • Outreach activities also include verbal interactions with members to establish relationships necessary for engagement (or re-engagement) and active case management. • Activities conducted prior to engagement (or re-engagement) are considered outreach. • Activities that continue after the member agrees to participate in the Health Home should not be included in the counts for this element. • Even though the Health Home cannot bill for services during the second three months ( hiatus) of the Outreach Period, any activities undertaken during this time to engage the member should be documented.

Outreach Effort versus Interventions • HH CMART is intended to capture information about the processes involved in care management service delivery and about the amount and type of care management services the person received • Outreach and Interventions capture different pieces of information and are used differently in analyses • Outreach for the Health Home population is critical and often resource intensive. The OutreachEffort element is intended to capture the work involved in locating members, establishing relationships and engaging the members. OutreachEffort includes attempts made to locate and engage members. This data will be used by the Health Home Program to quantify the effort needed to engage this type of complex individuals. • Interventions are quantified AFTER the member engages in care management and only completed interventions are counted. Interventions (mode #21-23) and care management services (category #30-34) quantify the care management services a member received. This information is used in analyses looking at correlations between the quantity and types of services provided with changes in utilization, cost and outcomes. • The data captured in the HH CMART is not intended to support billing or to verify billing.

  7. First Report Due – 2012 FACT-GP Assessment & HH Functional Questionnaire Data

  8. 2012 Submission Data Requirement What to Submit • FACT-GP Assessment / HH Functional Questionnaire data from calendar year 2012 is due Monday, May 13, 2013 • This first report will require only 2012 data collected for the FACT-GP Assessment / Health Home Functional Questionnaire for current members active in 2012 or for those that are now ‘case closed’ but were active in 2012. • After the member completes the FACT-GP Assessment /HH Functional Questionnaire, the answers are scored using the Scoring Guidelines for the FACT- GP /Health Home Assessment and FACT - GP and Health Home Questionnaire Scoring Sheet.

  9. 2012 Data-Excel Spreadsheet Template • 2012 data submission includes only elements # 2, 4, 7 and 35-49 of the Technical Specification’s document. • Below is an example of the 2012 partial Excel spreadsheet template showing the required elements for the 2012 FACT-GP Assessment / HH Functional Questionnaire data file due May 13, 2013. How to submit • 2012 data will NOT be entered into or require submission of the HH-CMART database tool. 2012 assessment data & scores will be entered into the above Excel spreadsheet template and submitted by the lead Health Home via the Health Commerce System (HCS) • Health Homes will send file using the HCS Secure File Transfer application to ‘Laura Morris’
