  2. Guidelines • Use EZPO version • Purchasing Limits: - $10,000 for Perkin Elmer, Amersham, and ICN for Fisher (not ICN itself) OR - $2,500 for all other vendors • Requisitions for amounts greater than these limits MUST be approved manually

  3. Guidelines Cont… • Any requisition for radioactive material must be approved by the Radiation Safety Office (RSO) • Failure to comply will result in corrective action, including suspension of permit for the extreme case of repeated offenses. • Once approved by the RSO, the requisition needs to be approved by the user’s department in order to generate an EZPO number.

  4. Ship To Information • Ship to the RSO’s Address! • Ship To Name: RSO/Primary Investigator’s name • Ship To Address: Radiation Safety Office 722 West 168th Street 4th Floor

  5. Line Items • CCODE is 5960 for radioactive materials. • State radioisotope name, compound name, and activity. • If all three are not included, the requisition will be rejected. • For contract orders, specify the contract period: start/end dates.
