Aetna Plan Offerings and What’s New? • Plans offered through Aetna for 2014: • 80% PPO Plan • Green Health and Savings Plan (formerly Bayer Health & Savings Plan) with Aetna pharmacy • FSA and Limited Purpose FSA • The 80% network Plan and Green Health & Savings Plan include coverage for Women’s Preventive Health services as a result of Healthcare Reform Changes

  5. Preventive Care – Covered 100% • Adult Routine Physicals • Well Child Exams • Routine Mammograms • Routine Gynecological Exams / Pap Smear • Routine Colonoscopy • Routine Prostate Specific Antigen / Digital Rectal Exam

Women’s Preventive Care – Covered 100% • Well-Woman Visits • Gestational Diabetes Testing • Prenatal visits during pregnancy • Breastfeeding Pumps and Supplies • (limits and frequencies apply - covers 1 electric breast pump in a 36 month period within 60 days of birth, 1 manual pump in a 36 month period within 12 months of birth, and 1 set of breast pump supplies on alternate years from receipt of pump) • Lactation Consultation • (limits and frequencies apply - covers 6 lactation consultations per rolling 12 months) • Contraceptives: • generic, FDA-approved contraceptive methods and devices • brand-name contraceptives when: • there is not a generic equivalent or alternative in the drug class (this includes NuvaRing® and the Ortho Evra® patch) • The provider indicates "dispense as written"; or • The member is approved by Aetna for a brand contraceptive during the step therapy or precertification process. • female condom, the sponge, spermicide and emergency contraceptives (when the provider writes a prescription and it is filled at a pharmacy) • Female Sterilization (Tubal Ligation)
