  2. Definition of Patient and Family Engagement Patient and family engagement is a partnership among patients, family members and healthcare providers. This team provides knowledge, insight and viewpoints for: • individual care • the design, implementation and evaluation of programs and policies Engagementconsiders the values and needs of patients, healthcare providers and the community. Patient: intended to mean all clients, patients and residents Family: A family is a group of individuals who are connected by kinship, affection, dependency or trust. The family is defined by the client. (from Saskatoon Health Region)

Overview • Not a legal document • Meant to act as a personal communication tool • To be completed primarily by the patient to enhance patient engagement, keep track of accurate history (facts, meds, surgery dates) and help the patient remember their care journey. • Contains medical information to be shared between patients and any health care provider as desired / appropriate • Includes tips on how to use the document
