What is Breast Density? The normal breast has two types of tissue: (1) Fatty and (2) Fibroglandular. On a mammogram fatty tissue appears black, while fibroglandular tissue appears white. Density refers to the proportion of fibroglandular to fatty tissue in a woman’s breast. Women with more fibroglandular tissue relative to fatty tissue on a mammogram are referred to have dense breasts (Categories C & D). How do I know if I have dense breast tissue? Only a mammography examination can determine if you have dense breast tissue. Other imaging techniques do not differentiate between fibroglandular and fatty tissue – they appear the same. It is important to know that dense breast tissue is not constant and may change over a woman’s lifetime. Women who are more likely to have dense breast tissue: *Are typically younger (for example, in their 40’s and 50’s) *Are premenopausal *Take combination hormone therapy to treat symptoms of menopause Some studies also suggest that diet and life style may affect the breast tissue density, although these are not definitive. Why is it important that I know my breast density? Breast density is like a double-edged sword. Not only does it increase the risk of breast cancer 5 fold relative to women with low-density tissue, but it also hides cancers (which also appear white) on mammograms leading to lower accuracy of mammography. In fact, 42–50% of cancers are missed in women with dense breasts due to lesion obscuration. In some instances in the USA, mammograms have missed breast cancers the size of a tennis ball. These occurrences lead to the passage of a National Law in 2019 that requires radiologists to inform women if they have high breast density, so they can consider alternates to mammography for breast cancer screening. Breast Cancer Screening for women with dense breast tissue: Other imaging modalities like ultrasound or MRI are not affected by high-density tissue. However, these imaging techniques can detect benign changes in breast tissue resulting in false positives and unnecessary biopsies.
