5. As we have prepared to welcome the GMC this academic year we have been reflecting on the influences summarised here on our on-going curriculum development. The new Edin medical curriculum is 10years old this summer - an ideal milestone for looking back over the events that led up to the radical new programme, esp with PDP PPD and Portfolios and PPD.
Factors that influenced our thinking that led to the introduction of PPD theme and why we created portfolios. These factors also make us consider future directions. The resulting educational discourses have thrown up tensions and queries, possibly questions to which others already know the answers. Can be aided by a mentor Moving onto Professionalism and PPD
IN Edinburgh one of the key ideologies in setting up the new curriculum was to encourage PPD. First and foremost PPD is a process driven by the learner for personal satisfaction and effective practice. The self-appraisal and goal setting is based on models of mentoring developed within GP as described by Freeman in XXX. But what of professionalism I have included it since it often goes hand-in-hand with PPD though again it was not mentioned as influential in determining Edinburghs new curriculum.. Eraut refers to PPD as an ideology and professionalism as a set of attributes. The RCP (Nov 2005) also described Professionalism as a set of values behaviours and relationships (listed above) that underpin the trust the public has in doctors.
This looks like the RCP response to Dame Janet Smiths report on the 5th inquiry into the Shipman murders in Jan 2005. Moving onto Professionalism and PPD

7. Professionalism Although by 2003 the GMC was suggesting a truly appraisal-based approach to revalidation with few detailed standards, Dame Janet Smiths report has resulted in a swing back to favouring a performance-based review system with papers from CMO Sir Liam Donaldson (2006) Good Doctors, Safer Patients and the DoH (2007) Trust Assurance and Safety strengthening that direction.
So what should our emphasis be in PPD?
How much can we permit it to be about the individual in medicine rather than the individuals performance in medicine?
Are portfolios and appraisal the right tools to measure Professionalism and if so how?
And if we use A portfolio as evidence of professionalism how else can we nurture the individual in a way that permits them to say Im not sure, Ive done things wrong, Im not good at and I want to do XXX because it fits well with the rest of my life.Although by 2003 the GMC was suggesting a truly appraisal-based approach to revalidation with few detailed standards, Dame Janet Smiths report has resulted in a swing back to favouring a performance-based review system with papers from CMO Sir Liam Donaldson (2006) Good Doctors, Safer Patients and the DoH (2007) Trust Assurance and Safety strengthening that direction.

15. Here are the current electronic formats of the 2 major portfolios in the Edinburgh curriculum. The lower MBChB Portfolio demonstrates some of its 23 items that a student will electronically upload and the upper picture is a screen shot of the home page of a students PPDPortfolio.
I set about trying to understand the sequence of events and influences that brought about these innovations by speaking to key members of Faculty staff and inspecting a range of documents.

40. In summary a schematic representation of the Edinburgh Portfolios .
I am reassured that the Learning and PPD portfolios have different but complementary functions (Propositional Knowledge and Performance).
Need to ask further questions about whether students come to us as SDL or if we can make them so wherever they start from.
To encourage students understanding of the career that lies ahead
Try to develop self-assess skills and advise of the difficulties surrounding this and self-efficacy.
We need to look more carefully at the reflective practice of experts and create advice & appropriate exercises for students.
Continue to develop a mentoring and developmental approach to appraisal that guides students to take control of their own development and career and to judge themselves against professionalism standards as one PART of the process.
Find alternatives to the PPD Portfolio and appraisal of ways of ASSESSING Professionalism. In summary a schematic representation of the Edinburgh Portfolios .
