Objectives & outcome data • Developing measurable Outcome Objectives: • Objective 1: Through collaborative efforts implement a Diabetes Self-management education and awareness program. • The target population will include diabetics and their family/ support person. Network with IHS, Tribal Behavioral Health Services, and Tribal programs. Present 1 session a month for 12-months. • Start the implementation of the Partners in Care curriculum for target population. Evaluation: By April 2003 plan the implementation of the curriculum. Network with other agencies. The target population to be comprised of newly diagnosed individuals who are ready to do self-management and a family member who takes part in the individual’s daily care. Schedule training for new staff members and interested community members to implement curriculum.
Interim & quarterly reports • Review grant reporting requirements included in application/ request for proposal • Generally: • Need to justify outcome of funding in all reports – need data to support entity is meeting stated goals & objectives • Use workplan to complete these reports • Use budget justification to track funding in coordination with identified goals & objectives • Use baseline data in reports to “measure” or track grant progress in meeting goals & objectives
