Definition Planning for Change in Long Term Care • Help with everyday activities of living (such as bathing, eating, dressing, getting to the bathroom, fixing a meal, shopping, cleaning) • Can be in one’s home, in a nursing home or group home, at one’s school or workplace, or anywhere else • For people of all ages who need help because of long-term disability Historically Long Term Care was…

Current Planning for Change in Long Term Care • The Department of Health and Family Services will foster the statewide development of comprehensive long term care and support systems that maximize independence, recovery and quality of life, while recognizing the need for interdependence and support. Goal of Current Redesign

Current Planning for Change in Long Term Care • The redesigned system will provide individuals and families with meaningful choices of supports, services, providers, and residential settings, as long as such care or support is necessary, meets an adequate level of quality, is cost-effective, is consistent with the individual’s values and preferences, and can be provided within available resources. Future Planning for Change in Long Term Care Care Management Organization –Provides help with managing services –Care management team includes at least a nurse, a social worker and the member (or guardian) –Manages the funding –Develop and manage a comprehensive network of services – choices in every category –Accountable for quality Family Care

  7. Future Planning for Change in Long Term Care Aging and Disability Resource Centers ADRCs ADRC Subcommittee formed with representatives of contract service providers, Aging Focal Points, disability groups and planning partners.

Future Planning for Change in Long Term Care Aging and Disability Resource Centers (ADRCs) Mission Statement The mission of the ADRC is to support seniors, adults with disabilities, their families and caregivers by providing useful information, assistance and education on community services and long-term care options, and by serving as the single entry point for publicly funded long term care services, while at all times respecting the rights, dignity and preferences of the individual.

Future Planning for Change in Long Term Care Aging and Disability Resource Centers • Dane/Rock County ADRC Goals: • Welcome the whole community to an attractive, accessible, non-threatening facility and services. • Reach and serve elderly people and people with disabilities, regardless of their income health condition and long term care needs. • Provide outreach and services to young people (age 17 ½ years) with disabilities as they transition from the school system to adult services.

Future Planning for Change in Long Term Care Aging and Disability Resource Centers • Dane/Rock County ADRC Goals: • Provide reliable and objective information about a broad range of community resources of interest to elderly people and people with disabilities. • Enable people to make informed, cost-effective decisions about long term care. • Delay or prevent the need for long-term care services and/or public funding for them. • Provide information and assistance to promote health and independence.

Future Planning for Change in Long Term Care Aging and Disability Resource Centers • Dane/Rock County ADRC Goals: • Serve as the single entry point for publicly funded long term care. • Identify people at risk and with needs and connect them to needed services. • Help adults access needed services through advocacy and assistance.

Future Planning for Change in Long Term Care Aging and Disability Resource Centers • Progress to Date • The subcommittee developed a 16-month work plan that falls into 5 categories • Develop a Mission Statement and Goals of ADRC • Design an operational model • Identify a physical location for ADRC • Develop fiscal data for operating ADRC • Work cooperative with IT, Data and Fiscal subcommittee on the Beacon database system for I&A

Future Planning for Change in Long Term Care Aging and Disability Resource Centers • Progress to Date • Two workgroups formed: Fiscal/IT Data and Staffing • Periodic meetings being held with the IT and resource database (Beacon) subcommittee • Developed an inventory of ADRC services provided by county government and provider agencies

Future Planning for Change in Long Term Care Aging and Disability Resource Centers • Progress to Date • Visited LaCrosse ADRC and invited Stephanie Stein, Milwaukee Dept. of Aging to subcommittee meeting • Hosted Barron County ADRC staff at meeting and contact has been made by some subcommittee members in Richland County

  17. Future Planning for Change in Long Term Care Aging and Disability Resource Centers Issues to be resolved Avoiding a conflict of interest between ADRC and CMO functions (creating a “firewall”)

  18. Key Tasks Planning for Change in Long Term Care Identifying a Governing Structure • Define relationship with state and between partners, including oversight, financial management, etc. Define Which Benefits Will Be Available • Family Care Model • CMO/Acute Care

Key Tasks Planning for Change in Long Term Care Subcommittees Formed to Address Key Needs • Aging Disability Resource Center (ADRC) Development • Communications/Public Involvement • IT/Fiscal • CMO Development Stakeholders will be involved in subcommittee work as appropriate for example to help determine a grievance system, development of quality assurance process and system access issues.

Future Meetings and Activities Planning for Change in Long Term Care Town Hall Meetings • Dane County - February 7th and May 21st at 2:00 p.m. and 6:30 p.m. at Alliant Energy Center • Rock County - January 8th and April 9th at the Rock County Job Center in Janesville Focus Groups • Invited participants help give feedback on specific issues and tasks related to Long Term Care planning process.
