Parent Signature: _____________________________________________ Instrument Choice: Please indicate below the instrument you would like to play. Please choose one. The instruments listed below are the only instruments instructed in band class. (Please note: students wishing to play saxophone are advised to try a different instrument first, and then switch to saxophone later) _____flute _____oboe _____clarinet _____alto sax (limited) _____tenor sax (limited)_____trumpet _____French horn _____baritone _____trombone _____percussion(see below) Students wishing to play percussion must have at least two years of piano instruction and a signature from their piano teacher. Band instruction is part of the school day. It is the responsibility of the parent/guardian to provide an instrument for instruction. The school has a very limited supply of school owned instruments. Band is a year long class, therefore we are looking for students and parents that can make that commitment!
