  1. Clinical Management of Dengue: Training for Clinicians May 9-10, 2011 Mercure Hotel, Vientiane, Lao PDR

Background • From 2005 to 2010 dengue cases had been reported in the entire Lao PDR. • Under the new MOU with the government, MID-BCC added dengue as an health topic for technical assistance to priority provinces. • In April this year, with the leadership of Vientiane Capital, the government launched a massive campaign to proactively identify, survey, and destroy breeding sites of mosquitoes in 9 districts. • To support the campaign, MID-BCC provided assistance in the production of IEC materials for schools, teachers, health service providers and the general public.

  3. Background • MID-BCC provided support in the conduct of this training on clinical management for medical staff in key hospitals in Vientiane Capital to strengthen their capacity in case management of dengue in hospital settings. • The training included a session on Interpersonal Communication (IPC) in Clinical Management of Dengue

  4. Objectives • To build and improve the capacity of health personnel in case management of DHF/DSS in main hospitals in Vientiane • To come up with a training agenda for dengue case management suited to provincial and district health personnel • To develop among the participants a core group of people who can serve as trainers for the provinces in the future.

  6. Participants • 15 participants were clinicians/attending physicians from the following: • Mahosot Hospital • 103 Hospital • 109 Hospital • Mittaphab Hospital • Setthathirath Hospital • 14 participants were key staff of NEIDCO, CIEH, and MOH

ASIA PACIFIC STRATEGIC PLAN 2008-2015 The Plan urges member states: (1) to ensure high-level political commitment so that adequate human and financial resources are made available for dengue prevention and outbreak response; (2) to strengthen health systems so as to improve diagnosis and case management; (3) to continue strengthening primary health care and health promotion in the prevention and control of dengue fever;

Interpersonal Communication “Effective interpersonal communication (IPC) between health care provider and patient is one of the most important elements for improving client satisfaction, compliance, and health outcomes. Patients who understand the nature of their illness and its treatment and who believe the provider is concerned about their well being, show greater satisfaction with the care received and are more likely to comply with treatment regimes. ”

  17. Government needs “ We need to continuously build the capacity of clinicians and health professionals. At the short – term , we can initially work with and prioritize provinces with the highest case fatality and morbidity rates, for example: . Salavan province, southern part of Laos.” Dr Bounlay Phommasack Director, NEIDCO

  18. Overall Recommendations • To ensure that provision of trainings to clinician will be sustainable, medical schools in Laos should be tapped. These medical institutions and their professors can assist and sustain the development or update of training curriculum and training materials. • The government should pursue a clear capacity building strategy for clinicians attending to dengue cases to ensure a more sustainable effort. • The government should develop “Champion” in dengue field (clinician) in Lao PDR in order to advocate the dengue case management activities • The government should pursue the development of Case Management Guidelines that is globally recognized and acceptable. • The government should identify and develop a hospital-based model for prevention and control. This hospital, being a model can also provide training to other clinicians.
