6. Progress to date • Consultation and focus groups • Health Need Profiles of all schools • Recruitment of staff completed • Analysis of identified need • Focus on transition times

7. Transition from Nursery to Primary • Whole school approach • Targeted intervention with identified vulnerable children • Supporting parents • Home link work if required • Support to families over the summer break

8. Transition from Nursery to Primary Focus of work on • Language skills • Pre-literacy skills • Narrative skills • Emotional health and wellbeing

9. Transition from Primary to Secondary • Whole school approach • Targeted intervention for identified vulnerable children • Support for parents • Home link if required • Support over Summer break if required • Focus on emotional wellbeing
