Take-home messages: • One-year mortality after discharge is high in patients admitted for acute heart failure. • More than half of deaths (56%) are due to heart failure and 16% due to sudden death. • Raised heart rate at discharge after acute heart failure is an independent mortality predictor and should be further taken into account in postdischarge management of heart failure patients.
