Purpose The purpose of this policy is to identify information that would be considered confidential and to establish guidelines on the treatment and disclosure of confidential information. Policy It is the responsibility of all employees to protect against the unauthorized disclosure of confidential information. This policy requires employees with access to student and employee information to refrain from sharing written, verbal or electronic information with others (both employees & non-employees) who do not require the information within the scope of their job or responsibility. This includes maintaining confidentiality in providing the student’s or employee’s name and/or any details that might identify the student or employee and relate him/her to a particular situation. Unauthorized Disclosure of Confidential Information Unauthorized disclosure of confidential information will result in serious disciplinary action, up to and including termination. CPS CANNOT SIGN ! • Students who require a surrogate are those who fall into one of the following categories: • The child’s parent cannot be identified. • The school cannot determine the parent’s whereabouts after having made reasonable attempts. • The child is a ward of the state, as defined in 20 U.S.C.§ 1402(36), and no parent can be identified or the parent’s whereabouts are unknown. • The child is an unaccompanied youth as defined in the McKinney-Vento Homeless Assistance Act. • The definition of a child’s “parent” includes: • The biological or adoptive parents • A foster parent • A legal guardian (but not the state) • An individual with whom the child lives with, including a grandparent, stepparent, other relative or other adult, who acts in the place of the biological or adoptive parent.
